Home Health Quality Initiative

IFMC works with Iowa’s Medicare-certified home health agencies to improve quality of care, using Outcome Based Quality Improvement. As Iowa’s Medicare Quality Improvement Organization, we offer support, expertise and resources at no cost to agencies working on Medicare quality improvement.

The focus of IFMC’s Home Health Home Quality Initiative is to assist agencies in providing the best possible care for their clients. These services include helping to reduce avoidable acute care hospitalizations; facilitating use of telehealth technology; and improving targeted performance measures.

IFMC’s Home Health Quality Initiative combines resources available to improve the quality of home health care with information for consumers about the quality of care provided by home health agencies available at www.medicare.gov.
