The data status of the patient. The two possible values are "complete"
and "incomplete". A value of "complete" indicates that all required
elements on all topics have been provided. A value of "incomplete"
indicates that at least one required element is missing, invalid or there are inconsistencies.

| Warnings |
Indicates if there are inconsistencies in the LDL-C information or diabetes elements.
"No" is displayed if there are
no inconsistencies, otherwise it displays "Yes".

| Locked By |
Indicates the username of the user who is currently modifying the patient information.
Only the user who has a lock on the record is allowed to modify it. If
no one is modifying the record or if the current user is the one modifying the record,
the displayed text is "Unlocked". See

Indicates the date and time the record was last updated. See the topic on

| Last User |
Shows the username of the last user who saved the record last. See the topic
on Log Viewer for information on how to trace
user activities. |
| Topic |
The name of the disease topic or preventive care measure. |
| Dx |
Indicates whether the patient has been diagnosed with the disease based on the information
from claims. Note that it is possible for a patient with a diagnosed disease
not to be ranked under that disease
topic. This is especially true for patients under PC-1 where it is expected
that the available number of patients eligible will be over 327. The Dx column
of all PC measures (not the PC condition) will always be blank as these are not
conditions. |
| Rank |
Indicates the rank number of the patient under a topic. Possible values are
"-----" (the patient is not ranked) or 1 to 327. If the patient is not ranked
under a topic, the tool will not allow that topic to be abstracted for that patient.
The PC Rank will always be "-----" since the patients are ranked
by individual PC measure rather than the PC condition as a whole. |
