Database Info

This section of the form displays read-only information about the selected database.  It is located below the Patient Status.  Below are the different parts of this section. Part Description Perf. Year Indicates the name of the performance year.  Possible values are "Baseline", "Year 1", "Year 2" and "Year 3". Min. Date Indicates the minimum acceptable date.  Any visit dates or lab result dates provided in this database that is before the minimum date are considered invalid. Max. Date Indicates the maximum acceptable date.  Any visit dates or lab result dates provided in this database that is after the maximum date are considered invalid.
