Abstracting Visits
 
The Heart Failure (HF) and Preventive Care (PC) topics include every-visit measures HF-3 and PC-1 respectively.  For every-visit measures, all visits within the performance year are gathered and not just the latest results.  These visits are added and modified in the grid and most of the data is synchronized.  The first row of the grids under HF-3 and PC-1 allows users to add a new visit.  See the topic on synchronized elements for more information.

If a date is added that is before the minimum date or after the maximum date of the performance year, an error message will appear.  To cancel the changes while adding or editing a visit, hit the Esc key.

Grid Columns
These are the columns in the grid under HF and/or PC-1.  Each row in the grid contains one visit and the grid contains all the visits for each patient.  All columns, except for Weight and BP, appear on topics HF and PC-1 and are automatically synchronized by the tool.

Column Description
Pre-filled This column is automatically set by the tool and cannot be modified by the user.  A check in this column indicates that the visit is pre-filled.  All user-added visits will not have a check in this column.
Visit Date Indicates the date of visit in MM/DD/YYYY format.  Duplicate known visit dates per patient is not allowed.  Multiple unknown dates (visit date is "X") per patient is allowed.
Weight This column is only available under the HF topic and indicates if a weight was taken during a visit.
BP This column is only available under the PC-1 topic and indicates if a blood pressure was taken during a visit.
Invalid Indicates if the visit is invalid.  Only pre-filled visits can be marked as invalid as their visit dates cannot be modified.
Why Invalid Describes the reason for marking a visit as invalid.  This is optional and should only be filled if the pre-filled visit is marked as invalid.

Rules
Below are the rules applied when modifying visits.

Pre-filled Visits
  • These are visits that were added to the database before it was sent to the practice.
  • These visits have a check in their "Pre-filled" column.
  • The date cannot be modified.
  • The visit can be marked as invalid.

User-added Visits
  • These are visits that were added by the users.
  • These visits do not have a check in their "Pre-filled" column.
  • The date can be modified.
  • The visit cannot be marked as invalid.

Visits marked as Invalid
  • The other fields in the same row are not validated.  It is possible to leave the other fields blank.
  • The reason for marking it invalid will be accepted but is not required.

Visits Not Marked as Invalid
  • The visit date must be a valid date or it must be an unknown date (“X”)
  • If the visit date is a known date then:
  • It must be on or later than the minimum date of the performance year.
  • It must be on or before the maximum date of the performance year.
  • Duplicate visit dates are not allowed
  • The date must be in MM/DD/YYYY format.
  • If the visit date is an unknown date then:
  • One or more unknown dates may be added per patient.
  • “X” must be used to represent an unknown date.

Adding and Editing
  • There is no limit to the number of visits that may be added per patient.
  • The Pre-filled column cannot be modified.  All pre-filled visits will have a check in this column and all user-added visits will not.  This column is maintained and set by MCMP-PAT.
  • All new visits added by users will not have a check under their Pre-filled column.
  • The visit date, weight measurement for HF visits and BP measurement for PC-1 patients are required if the visit is not marked as invalid.  If any of these fields is missing, the changes to the visit record will not be saved.
